Working Papers Procedures
An employment certificate, often referred to as "working papers", is required for minors under 18 before they may begin work. High school graduates, minors who work for their parents, and minors who do industrial homework are included in this group. Click here to view the NYS website

Complete Parts I,II & III of the attached application - required information needed:


Minor name, address, date of birth, parent signature.
Complete the Physical Fitness Certification form (physical must have been given within 12 months prior to issuance of the employment certificate)


Bring completed application and proof of physical to the Counseling Center.
The applicant must appear in person to obtain the working papers. Working Papers are issued during regular business hours.
*Part II - Evidence of Age for students attending Hamburg High School

The student’s school record will be used to verify age 

**Part III - Certificate of Physical Fitness for students attending Hamburg High School
Any student having a physical within the last year that was recorded by the High School Health Office, will be acceptable as proof. If there is not a current physical on file, proof from the student’s doctor office is required. 
Minors who do not attend Hamburg High School must bring in the following:

1.  Completed application with minor name, address, date of birth, parent signature and current physical (physical must have been given within 12 months prior to issuance of the employment certificate)

2.  Proof of Evidence of Age (Part II) and Certificate of Physical Fitness (Part III)
3. Bring the above paperwork to the Counseling Center. The applicant must appear in person to obtain the working papers.
